Creating Your Own Survey
Directions: For this assignment, you will create an original survey using polldaddy.com and publishing it on our site. The main objective of this survey is to collect data that will be used to build an infographic.
Task #1: Determine the topic for your survey.
This can be about almost anything (how students travel to and from school, eating habits of teens or summer travel destinations), but keep in mind the point is to enhance storytelling, so your data needs to tell a story. Use your topic to give your survey a title.
Survey topic & title – 3 points
Task #2: Create a custom message.
Inform readers what your survey is about and why they should complete it. Be specific about how the information will be used, and keep it brief.
Message to reader – 5 points
Task #3: Add questions.
You are required to use a minimum of THREE types of questions. Include one question type that allows you collect information about the respondent’s demographics: gender, age/grade, etc. Keep it appropriate!
Your survey must be a minimum of FIVE questions, but no more than 10.
Survey questions – 20 points
Task #4: Customize your survey.
Remember, you want the survey to stand out, but don’t go overboard. Make a choice that is consistent with the look and feel of our site.

Task #5: Share your survey.
Copy and paste the JavaScript code into Google classroom. Mr. D will use these codes to preview each survey before it is published.
